KCC pilot project transforms care in the community

Kent County Council (KCC) have set up their first self-managing domiciliary care team based in Ashford.

The team is a mixture of care workers, enablement support workers and occupational therapists who will be adopting the principles of the Buurtzorg model.

KCC is one of the partners working on the Transforming Integrated Care in the Community Project (TICC) and are working in partnership with a range of organisations across France, Belgium, Holland and the UK (Kent Community Health Foundation Trust, Medway Community Healthcare, Public World).

The TICC project is a four-year project, that aims to bring the Buurtzorg model of home care to the partner countries. The project has been approved and funded by the Interreg 2 Seas Programme 2014- 2020 (co-funded by the European Regional Development Fund).

Buurtzorg teams are responsible for managing the delivery of care to their clients and for managing themselves as a team. KCC’s Ashford South Neighbourhood Care Team was launched in July and will be working to the principles of the Buurtzorg model.

Anne Tidmarsh, Director of Partnerships at Kent County Council, said: “We are very pleased to be part of the TICC project which continues to effect real change as it works toward improve practice in integrated community care.

“The learning we take from the pilot projects will allow the creation a transferable blueprint for new services that will benefit Kent communities and others across Europe.”

KCC will be holding a launch event for the Ashford South Neighbourhood Care Team on Thursday 6th February (09:30- 12:30) at Farrow Court which will include an overview of the service and the opportunity to meet the team and ask them questions.

Places can be booked by email (TICCTeamSupport@kent.gov.uk) or by phone 03000 422 357.
